ElderNet serves people of all ages, especially frail older adults or younger disabled persons with low to moderate incomes who reside in Lower Merion or Narberth. Through its care management program, licensed social workers and coordinators support community members and their families as they navigate changing lifestyles. In-home and community-based services provide residents the assistance they need to remain independent. These services include conducting detailed assessments, providing referrals and help with public benefits applications, and offering emotional support through life’s challenges. As a result, community members in need gain improved access to healthcare, financial stability, and greater food security.
